    First-grade enrolment

    The form is a preliminary application for your child to join first grade. Based on it, we will prepare a document for you to sign: an application (child from the school's catchment area) or a request (child from outside the catchment area).

    The signed document must be submitted to the school office by the deadline given in the enrolment calendar. Sending the form alone does not mean admission.

    Child's details

    As in the child's birth certificate or ID card.

    If the child does not have a PESEL number, please enter the series and number of another document in the notes.

    Child's home address

    The address determines whether the child lives in the school's catchment area.

    Will the child use the after-school club?

    The after-school club is open from 6:30 to 17:00. You will submit the registration card in September.

    Will the child have school lunches?

    For example, siblings at our school or a request to be contacted. You will give health information to the form teacher in September.

    Information on data processing

    The data controller is Szkoła Podstawowa nr 7 im. Marii Konopnickiej w Zielonogaju. We process the data in order to carry out the admissions process, on the basis of Article 6(1)(c) GDPR in connection with the Education Law Act (Prawo oświatowe).
