After-school club enrolment: enrolment form and collection authorisations
We accept supplementary enrolments for the after-school club until 12 September. An enrolment form submitted in June remains valid and does not need to be confirmed after the holidays.
The after-school club enrols children from pre-school classes and students in Grades 1–4. The club is open from 6:30 to 17:00, and students in Grades 5–8 can stay there without enrolling while waiting for activities or the bus.
If there are more applications than places, the head of the after-school club, Katarzyna Lewandowska, informs parents of the decision by 15 September. Usually there are enough places for all children who apply.

Getting the form
The enrolment form and the collection authorisation can be downloaded from the website or picked up at the after-school club and the school office.
Filling in and signing
The form is filled in and signed by a parent or legal guardian. Please include all current phone numbers.
Handing in the form
Hand in the form at the after-school club or the school office. You can send a scan by e-mail and bring the original within a week.
